Job Description :

The Registered members of the nursing team are expected to provide leadership and direction to the PSW members of the team by always providing guidance with dignity and respect. The PSW team is essential to the provision of care on the unit, therefore the Registered team members will use their knowledge, skill, and judgment to develop a culture of teamwork and camaraderie.20 bed unit with Rn and PSW support.

Qualifications and Educational Requirements :

• Current Annual Registration as an RPN with the College of Nurses of Ontario.
  

• Recent Criminal record check (Vulnerable Sector) and 2 Step T.B. Test/Chest x-ray results required, along with current CPR Level C and First Aid documents and valid eligibility to work in Canada.
  

• Understanding of the requirements under the Long-Term Care Act, CNO Standards, and other regulations relevant to Long Term Care and Community Health.
  

• Good verbal and written communication skills, using tact, diplomacy, and empathy to share difficult information with a sensitive and compassionate approach, high level of detail orientation.
  

• Ability to work independently and in a team based, collaborative fashion handling multiple tasks safely and efficiently.
  

• Basic computer skills.
  

• Demonstrated ability to handle critical situations in a practical, logical fashion.
  

• Gentle Persuasive Approach, Non-Violent Crisis Intervention, and other relevant training an asset.
  

• Previous experience working with the senior demographic a definite asset.
  

• Ability to meet the physical requirements of the job – e.g., long periods of standing/walking, pushing/pulling, and physical lifting capabilities, and ability to help clients to reposition and transfers.
  

Job Duties :

• The Registered Practical Nurse will assist/manage all PICC lines, Central lines, initiation of peripheral lines, and IV medications on the unit.
  

• RN’s and RPN’s will administer all IM, Sub-cute, and P.O. medications. Topical medications can be administered by the PSW once they are educated on their application.
  

• All documentation will be done by the Registered staff.
  

• All assessment will be done by registered staff: All admission assessment, (V/S and weight will be done on the patient’s bath day (weekly)). PSW’s will be trained to measure patient weight on bath day.
  

• Every shift a Respiratory Screening assessment including auscultation of the patient’s chest, a full set of vital signs, a CAM assessment and any focused assessments for change in condition.
  

• The Registered team will be a able to delegate certain tasks to the PSW’s. Application of prescribed ointments will be provided by the PSW’s after instruction is given from the Registered staff.
  

• The Registered staff will follow up on any reports of changes in condition from the PSW in a timely manner.
  

• Provide direction, support, and delegation to unregulated employees as appropriate.
  

• RPN will use the On-Call protocol to contact the On-call physician under the following circumstances:
  

• There is a change in condition requiring medical intervention outside the scope of the nurse.
  

• The Primary Care physician is not available.
  

• Nursing resources will be found on the desktop for ease of access.
  

• All team members will observe the Infection Prevention and Control Protocols as recommended by Public Health Ontario. Any issues with IPAC protocols will be reviewed by the manager in consultation with the team. Training will be provided for all team members.
  

• All team members are expected to communicate with each other to give report before leaving the unit for break to ensure there is sufficient coverage on the unit. The staff are not permitted to leave the unit during their shift without first consulting with the Nurse Manager.
  

• Follow established Always Practices - Check in on patients every hour. Assess pain, repositioning, toileting, and other needs.
  

• Duties are subject to change to reflect the operational needs of the organization without prejudice.
